As a reminder, evidence of certification and evidence documenting your readiness to provide
service within 60 days of the thousands-block effective date must be submitted the same day
the initial thousands-block request is submitted in PAS. In addition, written approval from the
state must be submitted the same day the state waiver request is submitted in PAS.
