Thousands-Block (NXX-X) Confirmation Form
Part 4 (Updated)
This tip is being sent at the request of a couple of the state commissions. As a reminder,
the Thousands-Block (NXX-X) Confirmation Form Part 4 was updated on March 19, 2007 to
add the Company Name and OCN fields. The state commissions wanted us to send out a reminder to service providers asking them that when submitting manual Part 4 forms to the state commissions for overdue part 4’s, to use the new Part 4 form. The new Thousands-Block (NXX-X) Confirmation Form Part 4 with the new fields makes it easier for the state commissions to identify which company the Part 4 is for.
