As a reminder when completing the List excluded Code(s) or Block(s) in section D of the MONTHS TO EXHAUST and UTILIZATION CERTIFICATION WORKSHEET, applicants should only list those blocks and/or codes that were assigned by the administrator for the requested rate center and were activated within the past 90 days in the LERG. Any blocks and/or codes that were activated in the LERG after 90 days should not be included in section D as well as any blocks and/or codes that are not assigned to the requested rate center.
