Thousands-Block and Pooled
Code Applications
As a reminder, evidence of certification and evidence documenting your readiness to provide service within 60 days of the thousands-block/pooled code effective date must be submitted to the PA the same day the initial thousands-block request and/or pooled code request is submitted in PAS.
In addition, written approval from the state must be submitted the same day a state waiver request is submitted in PAS. Also, be sure to reference the PAS tracking number on the supporting documentation submitted with your request so that it can be properly matched to the request made in PAS.
