Supporting Documentation for Initial Thousands-Block and Pooled Code Requests
As a reminder, evidence of certification and proof of facilities documentation must be submitted to the PA the same day the initial thousands-block request and/or pooled code request is submitted in PAS. Thousands-Block Number Pooling Administrator Guidelines (TBPAG) section 4.3.1 outlines the acceptable certification and proof of facilities readiness documentation accepted by the PA for initial resource requests.
In addition, written approval from the state must be submitted the same day a state waiver request is submitted in PAS. Be sure to reference the PAS tracking number on the supporting documentation submitted with your request so that it can be properly matched to the request made in PAS.
